Duties Description As a Social Work Assistant 1 at Greater Binghamton Health Center, you will work with an interdisciplinary team and duties will include:

• Interviewing individuals and their families to gather background information
• Preparing case histories and determining eligibility for services
• Advocating and assisting individuals in identifying and obtaining services

NOTE:
1. If appointed, you may be required to become an enrolled Medicare provider; obtain and provide to your employer a National Provider Identifier (NPI) number issued by the National Plan and Provider Enumeration System (NPPES); and otherwise actively participate to the degree necessary to allow for your services to be billed through Medicare and Medicaid. If you are appointed and lose the ability to bill through Medicare and Medicaid, you may be terminated from your employment.

Minimum Qualifications Greater Binghamton Health Center is recruiting for Social Work Assistant 1 to provide case management and service coordination activities for individuals with mental illness. Candidates from outside or within State Service can apply.

Minimum Qualifications:
Candidates from outside or within State Service can be considered for hire under the Hiring for Emergency Limited Placement – Statewide (NY HELPS) program if they have a bachelor’s degree in a human service field and 1 year of social work experience.

Substitution: Master’s degree in social work can substitute for 1 year of specialized experience.
