Minimum Qualifications Candidates from outside or within State Service can be considered for hire under the Hiring for Emergency Limited Placement – Statewide (NY HELPS) program if they have a bachelor’s degree in a human service field and 2 year of social work experience.

Substitution: Master’s degree in social work can substitute for 1 year of specialized experience. OR

Candidates within State Service can be considered for a competitive class appointment if they are reachable on the current eligible list. OR

Candidates from within State Service can be considered transfer to a competitive class position if they have one year of permanent competitive or 55-b/55-c service in a title eligible for transfer under Section 70.1 of the Civil Service Law.

Duties Description PRIMARY DUTIES AND RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO:

The Waiver Unit is a key component to the core mission of our agency and is the hallmark of our transformation agenda. As such this candidate's primary function, although related to the Service Access Team, will be the primary contact for individuals transitioning from their local school district into adult services with OPWDD. Familiarity with Tracking and Billing System, CHOICES and ability to maintain databases and monitoring systems preferred. This employee will be responsible for the following:

• Complete EAAs and DDP2s based on conversation with family/individual transitioning out of their school district into services with OPWDD

• Provide Front Door Information Sessions on site, communities and in schools.

• Provide trainings, education and outreach to school districts in Brooklyn

• Work with district personnel in identifying upcoming graduates, schedule Front Door Sessions and assist with Service acquisition

• Track caseload using Front Door Spreadsheet and compile reports of transitioning students to interested parties.

• Attend meetings with school district personnel, voluntary providers, advocacy organizations and other State agencies

• Troubleshoot problems.

• Complete special assignments as needed

• The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies

• All OPWDD employees must be eligible and maintain eligibility for full and unconditional participation in the Medicare and Medicaid programs. Continued employment will depend on maintaining eligibility.
