Minimum Qualifications Metro NY DDSO is currently recruiting for a Rehabilitation Counselor 2 to provide rehabilitative services to individuals with developmental disabilities.

Candidates from outside or within State Service can be considered for hire under the Hiring for Emergency Limited Placement – Statewide (NY HELPS) program as Rehabilitation Counselor 1 if they have:

• Bachelor’s degree in a human service field and

• 3 years of professional experience providing vocational, psychosocial, or rehabilitative/habilitative services in an agency primarily serving individuals diagnosed with developmental disabilities, mental illness, or addictions.

• Substitution: Master’s degree in a related field can substitute for one year of specialized experience.

Candidates from outside and within State Service can be considered for a competitive class appointment if they are reachable on the current Rehabilitation Counselor 2 eligible list.

OR

Candidates from within State Service can be considered for transfer to a competitive class position if they have one year of permanent competitive or 55-b/55-c service in a title eligible for transfer under Section 70.1 of the Civil Service Law.

Duties Description PRIMARY DUTIES AND RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO:

1) Will be responsible for ensuring that Life Plans are received timely and contain all the necessary elements to support Medicaid billing and compliance.

2) Interfacing with Care Coordinators, Habilitation Specialist, Family Care Liaisons to ensure that Life Plans are received timely.

3) Act as Liaison with the CCO Agencies, Medicaid Compliance Liaison, and other entities.

4) Create tracking systems to maintain tracking of all Life Plans.

5) Provide training to residential and day program staff.

6) Complete CFR Projects, FIDA-IDD, and agency investigations.

7) Provide clinical supervision, instruction, training, and in-services to all Habilitation Specialists (Bronx and Manhattan), as well as review documents for quality: Life Plans, Staff Action Plans, Safeguard Summary Plans, Monthly Summary Notes, Gatekeeper Form, etc.

8) Conduct site visit observations to ensure that person-centered goals are being carried out as per the HCBS wavier documentation (ie. Life Plan and Staff Action plan)

9) Review EHR to confirm entries and documentation have been completed/uploaded.

10) Establish working relationships with CCOs and ensure Life Plans, (LP) are scheduled as required, create tickler system to maintain tracking of all Life Plans, contact applicable parties to reconcile any concerns noted during Life Plan audits, and interface with the Medicaid Compliance Officer and liaisons.
