Minimum Qualifications Open to the Public: This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). *The minimum qualifications through the NY HELPS are:
License and current registration as a Licensed Master Social Worker in New York State.
Must maintain a valid New York State license and registration.
Duties Description This Front Door Access Team member will be responsible for:
• Contact families coming through the Front Door to provide information and guidance on the process.
• Conduct Front Door Trainings for individuals and families.?
• Conduct intakes/assessments with individuals and families to determine appropriate service level.
• Complete Developmental Disabilities Profile 2 forms.
• Open EAA tool and enter identifying information.
• Meet with families and other key stakeholders to gather pertinent information and documentation. Process the information and submit service recommendations for authorizations.
• Track activities to ensure the timely progression of individuals through the Front Door process.
• Keep families apprised of progress through the Front Door process.
• Direct families to the Coordinated Assessment Process
• Approve Quality Review Team (QRT) 1 requests and submit QRT 2 requests for approval, when necessary.
• Review Home and Community Based Services (HCBS) packets before submitting to HCBS unit.
• Process Notice of Decisions.
• Process Level of Care Eligibility Determinations.
• Close EAA tool and engage in lost contact protocol, when necessary.
• Troubleshoot problems/issues with the Front Door Team.
• Participate in weekly Front Door team meetings and training, as scheduled.
• Participate in functions and activities occasionally, during evening hours and weekends.
• Complete mandatory training.
• Participate in special projects as assigned by supervisor.
SELECTION CRITERIA:
• Possess excellent and professional communication, public speaking, written and organizational skills.
• Ability to initiate and maintain working relationships with individuals we serve, families, advocates, voluntary agency staff and Non-Medicaid Service Coordination and Care Coordination Organization staff.
• Working knowledge and proficient use of a computer.
• Ability to work independently and with a team.
• Possess acceptable time and attendance work record.
