Job Description

The New York City (NYC) Bureau of Tuberculosis Control (BTBC) is committed to preventing the spread of tuberculosis (TB) and eliminating it as a public health problem in NYC. The BTBC Office of Epidemiology reviews and assesses exposures in residential and non-residential congregate settings; conducts investigations in these settings to identify, notify and evaluate contacts and assess transmission; analyzes genomic and epidemiologic information to detect, investigate and respond to TB outbreaks; and conducts data analysis and research to describe the epidemiology of TB in NYC and to inform TB prevention and control policy.

We are seeking a Care Coordinator to join our Network Epidemiology and Expanded Contact Investigations (ECI) Teams in the Office of Epidemiology, which work together closely to review suspected and confirmed TB cases, investigate TB exposures in congregate settings, and support care coordination for individuals exposed to TB. As an integral part of the team, this person will assist with case review, data analysis, and contact investigations in diverse site types across NYC, will be part of the decision-making process that leads to such investigations, and will support health education and linkage to care. The ideal candidate will be organized, have strong attention to detail, enjoys and has experience working with and serving the public, has familiarity with the healthcare system, and can work both independently and as part of a team.

DUTIES WILL INCLUDE BUT NOT BE LIMITED TO:

- Review and enter data in the Health Department's TB surveillance and case management system.
- Plan and conduct TB contact investigations in residential and non-residential congregate settings (e.g., worksites, schools, shelters).
- Communicate with patients, health care providers, laboratories, and others (in person or on the phone) to conduct interviews, collect relevant information, and arrange for follow-up medical evaluation and treatment.
- Assess results of investigations to determine if further expansion or public health intervention is warranted.
- Assist in conducting TB education sessions at sites where TB exposures have occurred.
- Conduct contact identification, notification, education, and care coordination.
- Support administration and implementation of TB testing events in community settings.
- Conduct TB testing, including specimen collection (phlebotomy) and specimen transport as needed.
- Conduct medical chart review.
- Other duties as required.

PREFERRED SKILLS:
- Knowledge of the health care delivery system.
- Experience working and communicating with patients and/or the public.
- Experience working and communicating with clinicians or other health care or social service providers.
- Experience with structured interviews, medical terminology, and conducting medical chart reviews.
- Experience with public health education.
- Good written and oral communication skills.
- Excellent organizational skills.
- Ability to work in a team environment.
- Working knowledge of Microsoft suite (MS word, Excel etc.).
- Experience working in the community in a social service, mental health or public health capacity
- Must be willing to work occasional evenings and weekends.

Minimum Qualifications

1. A baccalaureate degree from an accredited college and two years of experience in community work or community centered activities in an area related to the duties described above; or
2. High school graduation or equivalent and six years of experience in community work or community centered activities in an area related to the duties as described above; or
3. Education and/or experience which is equivalent to "1" or "2" above. However, all candidates must have at least one year of experience as described in "1" above.

Residency Requirement

New York City residency is generally required within 90 days of appointment.
